AVOID heart attacks, strokes, and kidney disease by getting your blood pressure and blood glucose under GOOD CONTROL!

Remote Monitoring Program
Healing Our Village has harnessed the use of a digital health engagement platform and individualized health
coaching to improve the self‐care management and outcomes of the type 2 Diabetes, cardiovascular disease,
and obesity. The goal is to improve medication adherence, glycemic and cardiovascular control.
Reimbursement for Remote Patient Monitoring ‐ Starting January 2019, CMS and some commercial payors now cover Remote Patient Monitoring

Health Wealth Safe and Healing Our Village will work together to facilitate initial patient enrollment, device delivery, device set up, text messaging, telephonic outreach, etc. ‐ $21 per patient ONE TIME BILLING
