Village Club was developed as a low cost option for “At Risk” and Vulnerable Populations. Village Clubtargets participants who are uninsured or under insured.  We encourage Health Plans and Employer Groups to purchase Village Club ($35.00/month or $365.00/year) for their members to ensure they are getting to their treatment goals.  Especially important for those members with Diabetes and Hypertension.
